Niantic wants to level the playing field for Pokemon Go players and they have had it with cheaters. If you are caught location spoofing, using any tricks, Pokemon Maps or try any other way to cheat/hack the natural gameplay, you will be permanently banned.
The company has added a new section to their support page which now says that instead of a temporary ban players will be permanently banned if found cheating. So any of these would be your shortest way of getting on the naughty list:
falsifying your location, using emulators, modified or unofficial software and/or accessing Pokémon GO clients or backends in an unauthorized manner including through the use of third party software.
However, if you feel Niantic was unfair in banning your account or your were falsely accused, you can appeal to the ban as well.
